Medication at school should be given at home whenever possible, however when dosage instructions require medication to be given during the day, the following requirements must be met:

SELF CARRY:

Students MAY NOT carry medications with them or self-medicate unless it is an emergency medication when it is age appropriate. Those medications that qualify as emergency medications ONLY include EpiPens, insulin, Diastat and inhalers. In order to self-carry these emergency medications, the clinic must have written physician orders to “CARRY AND SELF ADMINISTER” and the parental permission form on file in the campus clinic. Please download the appropriate form on the “FORMS” page or ask your Nurse/Nurse Aide for one.

PRESCRIPTION MEDICATIONS:

Prescription medication may be given at school if a parental permission to administer form is sent by the parent/guardian. The form includes specific instructions on how, when, and why the medication is to be given as well as if the student has any drug allergies. The medication must be in the original prescription bottle, recent and properly labeled with the name of the patient, doctor, drug, and specific instructions on the dosage. The bottle will NOT be transferred from home and school so please ask your pharmacist to split the prescription into two containers. OVER-THE-COUNTER MEDICATIONS:

A permission to administer form from the parent/legal guardian is required granting parental permission for the OTC medication to be administered at school. The form includes specific instructions on how, when, and why the medication is to be given as well as if the student has any drug allergies. Frequency and dosage may not exceed the directions listed on the bottle without documented physician orders. The medication must be provided by the parent and appear in the original unopened container with their name written on the box. OTC medications given for 5 consecutive days will require physician orders to continue. Please download the appropriate form on the “FORMS” page or ask your Nurse/Nurse Aide for one.

HERBAL REMEDIES:

Herbal remedies will not be administered at school unless there is a written physician order and it is in the student’s IEP that they are to have the treatment at school.
